With every follow-up order, you fill in an evaluation form. This is a short medical check the doctor uses to assess whether your treatment is going well and whether your dosage is still appropriate. Without a completed form, the doctor can't issue your prescription and your order won't be processed.
When does the form appear?
The evaluation form appears automatically in your portal once payment for your follow-up order has been received. You'll also receive a reminder email.
Where can I find the form?
Log in to your personal account. If the form is ready, you'll see a banner at the top of your treatment page. Click it to open and fill in the form.
What's in the form?
The form includes questions about your treatment results, any side effects, whether you'd like to adjust your dosage, and your general wellbeing. It usually takes 2–3 minutes to complete.
Fill it in quickly = a faster order
The sooner you fill in the form after payment, the sooner the doctor can review and issue your prescription. Preferably fill it in right after receiving your payment confirmation.
